The Healthy Heart Program at University Hospital(UH) launched on December 16, 2014 with the focus of helping heart failure patients improve their ability to manage their care and reduce the need for hospitalization. The program is a one-stop medical home for patients where they have a multidisciplinary team assigned to their care. With the guidance of the ambulatory care technicians, health educator, registered dietitian, cardiac physiologist, pharmacist, mental health clinician, clinical care coordinator, patient navigator, cardiologist, and APN, the program aims to find solutions to barriers associated with lifestyle, nutrition, exercise, medication, mental health and social issues. By focusing on tertiary prevention patients can gain back their independence while improving their quality of life. UH's heart failure program has been certified by The Joint Commission and offers tertiary care for all patients.
