The mission of Community Health Care, Inc. is to provide the communities we serve with excellence in patient-centered medical, dental and behavioral health care that is compassionate, affordable and accessible.

CHC was founded in 1975 to offer a medical and dental home for persons who could not access primary health care elsewhere in the community. We offer preventive, acute and chronic care, as well as laboratory, pharmacy, nutrition, and health education services.

Without CHC, many patients would rely on emergency rooms during crisis, or go without care altogether. In recent years, we have made great strides serving more than 37,000 last year. Our services are only possible because of the generosity of others.

We have a total of eight locations in the Quad Cities and Clinton, IA so we can provide geographically accessible care in addition to our low fees. For example, someone without health care insurance can see one of our providers for as little as $20.

In over forty-two years of service, we have provided a primary care safety net for those in our community who are most vulnerable.

Our services make an impact on everyone living in the Quad Cities as we employ over 300 people, keep patients from using the emergency room as their primary care provider, and we create a healthy environment that is good for children, parents and grandparents.

We are accredited by the Joint Commission on Accreditation of Health Care Organization. JCAHO is an independent accrediting agency who performs on-site surveys of CHC’s patient care, customer service, safety practices and patient privacy.

Patients with concerns about patient care and/or safety within CHC are encouraged to contact the CHC leadership team at 563-336-3000. If the concerns cannot be resolved by CHC, the patient can contact the Joint Commission’s Office of Quality Management to report concerns or register complaints about CHC by either calling 1-800-994-6610 or emailing complaint@jcaho.org.

This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n). Deeming is applicable to malpractice liability coverage only.
