CareSource was opened in 1997 by Gina Coccimiglio and Shawni Tanner with the plan of providing cost effective and efficient care to elderly people in their homes. As owners of CareSource, Gina and Shawni are dedicated to ensuring that patients receive the highest quality of care by employing more than one hundred of Utah’s finest people.

CareSource Home Health and Hospice is known in the community for providing high quality care to seniors in their home environment. We are a Medicare certified and state licensed home care and hospice agency. Home care services include skilled nursing, physical therapy, occupational therapy, speech therapy, medical social services, and home health aide services. The goal of home care is to bring a patient back to their previous level of functioning after an illness. Hospice services are provided to individuals who have a terminal diagnosis and their physician will certify that they have six months or less remaining. Hospice services can be provided anywhere the patient resides including homes, assisted living facilities, skilled nursing facilities, and even hospitals. Hospice is a way of providing care and not a place.

CareSource currently has Utah’s first and only dedicate hospice inpatient facility - The Residence. The Residence is unlike any other facility in the state for a number of reasons. First, it is the only state licensed and Medicare certified hospice facility in the state. Second, it has been designed unlike any other facility with a more home-like atmosphere. Instead of the long institutional hallways, The Residence has a gathering room with rooms extending off of it. Third, it is small enough to ensure the high quality care CareSource is known for. Our facility has the look and feel of a very high scale assisted living, while providing the high quality skilled nursing services of a skilled nursing facility or hospital.
