Family Health Teams are health care organizations that:
Include a team of health care professionals (i.e. physicians, nurse practitioners, nurses, pharmacists, social workers, dietitians and others) who work together to provide health care for the community;
Ensure that people receive care they need in their communities by providing better care, closer to home;
Focus on chronic disease management, disease prevention and health promotion and work with other organizations in the local community such as Public Health Units and Community Care Access Centres.
